• The equation references in the middle of page 37 should be as follows: Page 37 correction.

Chapter 3

• The variance computations at the bottom of page 141 should read as follows: Page 141 correction.
• The computations in equations (3.16) – (3.22) should be replaced by the following: Page 149 correction.

Chapter 8

• Equation (8.41) should be replaced by the following: Page 316 correction.

Chapter 10

• Equation (10.39) and the third line from the end of that paragraph should be replaced by the following: Page 464 correction.

• The first paragraph of page 324 states that the odds ratio is 2.70, but should instead say 2.09.
